Patient Referral
There are two ways to refer your patients to us.

1. Online Referral - You may fill out our secure Online Referral Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of patient data is one of our primary concerns and we have taken every precaution to protect it.

2. Download Referral Form - You may also download, print, then fill out a Paper Referral Form. Either email (info@425oralsurgery.com) or fax (425-961-9601) the form and give a copy to the patient as well.

Dr. Yi is an AHA certified BLS (Basic Life Support) Instructor! We can conduct BLS/CPR courses for you and your staff at our office. Call us for more information!
