There are two ways to refer your patients to us.

1. Online Referral Form. After you have completed the form, press the Complete and Send button at the bottom >

2. Download Referral Form - Download, print, then fill out the Referral Form. Either email (info@425oralsurgery.com) or fax (425-961-9601) the form and give a copy to the patient as well.
