Minor patients: parent/legal guardian required

If the patient is under 18 years old, a parent or legal guardian must complete and sign this registration form and be present at the appointment to review and sign all required treatment-consent, anesthesia-consent, and financial-responsibility forms. 

If a parent or legal guardian cannot attend, please contact our office before the appointment at 425-961-9600. Appointments may need to be rescheduled if required consent cannot be obtained.
